Thank you for your interest in SMART’s Lesson Activity Contest. Please read the Contest Terms and Conditions before completing this entry form. The Contest Terms and Conditions, along with the Lesson Activity Waiver, must be agreed to before you submit your lesson activity.
If you have already read the terms and conditions, please read the following Lesson Activity Waiver and complete the form. Once you successfully submit the form, you will receive a confirmation notice and the contact information necessary to send us your lesson activity by e-mail.

By submitting my lesson activity to SMART, I declare that such lesson activity has been developed solely by me and has not been copied from any other source, except where indicated and where the requisite consent has been obtained. I hereby assign all copyright (including all associated moral rights) in and to the lesson activity to SMART.
I understand that SMART may use my lesson activity (i) in any manner; (ii) in any media form whatsoever; (iii) without any compensation to me; (iv) without any obligation of confidentiality; and (v) with or without recognition of me, as SMART in its sole discretion may choose. SMART shall not be liable for any use or disclosure of my lesson activity or any associated materials submitted by me. I also understand that SMART may choose not to use my lesson activity. In any case, SMART is under no obligation to return any materials submitted by me as my lesson activity and any associated materials become the property of SMART upon submission by me.
